As it was already pointed out: They did exactly that when Mayim hurt her hand during a car crash some years ago. And they did it so well that a lot of people didn't know/notice. Back then Amy and Sheldon were no more than couch sitting buddies, Plus Mayim didn't have to wear that brace as Johnny does. It's not really worth the effort it would take to try to hide the brace. But just having Leonard hand his beer to Penny to open (and her look) would be funny. Physically inept Leonard is part of his Schtick which is why it works. But sure they could hide it for an episode or two. But why? In the most basic 4A group scene Leonard almost always sits on the right exposing his left hand.
