He stated that he tells people they hooked up and that she was on his list of sexual partners.  Both imply that they actually had sex, when they didn't.  It's quite reasonable for Penny to tell the full story.   Not to mention, if he doesn't want Penny to say anything, then he shouldn't either.

Where exactly does it say sex in that comment?  It says love burned too bright, but not sex.

Besides, I took the not telling anyone as she wasn't going to mention he couldn't control himself.

Also, extremely bad form on his part---it's been a while since I've watched the episode, but didn't he want her NOT to disclose certain aspects (performance related) of the situation?  (again, I have not seen it in more than 2 years...)

I dont think Penny agreed Raj could. I think she agreed he said their "love" burnt too fast , as it did not even had time to start. I dont see any sex confirmation there.

I disagree, a hand job is sex and that is basically what Raj got from her, even if it was by putting his protection.  Saying they didn't have sex is like Bill Clinton saying he didn't have sex with Monica to me.

Sheldon was not her doctor and if her doctor groped her boob for a shoulder dislocation, yeah she should have told someone because it is inappropriate.  The thing is she never told Leonard any of it, and seemed fine to keep it in the dark until it just came out, so she lied by omission in my book.

The "promise" to Raj, which BTW illustrates again how excessively soft-hearted Penny is, has no weight compared with her relationship with Leonard. Penny has every right to tell Leonard anything, and has no obligation to anyone to keep secrets  from Leonard. Generally she would be sensible to tell L everything as she is actually quite naive (her "street smarts" seem to be a collective delusion) and Leonard should advise her when people have taken advantage of her.

Sheldon had his eyes closed as per her instructions. I sure would hope that a doctor wouldn't treat a patient by fumbling around with closed eyes, subsequently any boob groping by a doctor would be intentional.

He clearly had his eyes closed when he tried to help her with the top which is what we actually saw. We didn't see him peeking at her butt where her tattoo would be - presumably that happened when she had her back turned to him for a moment.

But the question wasn't about whether they had sex, it was what Penny agreed Raj could tell anyone.  She agreed he could say (... that their love burned too bright...), not that they had sex.  Raj was telling people they had sex.  And, from what I get from Penny's comment, she agreed not to say anything about Raj's performance issues.  She hasn't as far as I know, but That doesn't mean she can't say something to correct Raj's inacurate comments, or insinuations, about what else happened.
